PRP Treatment Record Template
A printable record for platelet-rich plasma treatments, covering the blood draw, how the PRP was prepared, how it was delivered, and how the client responded, so a series can be compared session to session.
Record the preparation, not just the treatment
PRP is prepared from the client's own blood at the visit, so the preparation is part of the treatment. Record the volume drawn, the collection system and kit lot, the centrifuge settings, and the volume of PRP produced. Two sessions with the same technique can differ if the preparation did, and the record is the only way to tell.
Then record the delivery: injected, microneedled, or both, which areas, how much per area, and any anaesthetic used. For hair and facial series alike, baseline and follow-up photos taken under the same lighting are what make a result assessable.
