Laser Tattoo Removal Treatment Record Template

The tattoo is the baseline
Record the tattoo's size, location, colours, age, and whether it was professional or amateur, then photograph it in the same light at every session. That baseline is what lets you and the client judge progress, which in tattoo removal is slow and easy to misjudge.
For each session, record the wavelength, spot size, fluence, and passes, plus the endpoint you saw. Settings change between colours, and the interval between sessions matters, so both belong on the record.
