Laser & IPL Treatment Record

A printable treatment record for laser hair removal, IPL, and other energy-based treatments. It captures the device settings per area, the pre-treatment checks, and the clinical endpoint, so each session in a series starts from the last one's settings.

Settings belong in fields, not sentences

Laser treatments are delivered as a series, and each session is adjusted from the previous one based on the settings used and how the skin and hair responded. Record the device and wavelength, handpiece or spot size, fluence, pulse width, repetition rate, and cooling for each treated area. Written as a sentence, those numbers are easy to misread; in a table they can be compared at a glance.

Record the pre-treatment checks as well: skin type, recent sun exposure or self-tanner, photosensitising medication, a test patch if one was done, and eye protection. Then note the clinical endpoint you were aiming for and whether you reached it.

Template from zdrovia.ca/templates/laser-treatment-record/ · Adapt to your college's documentation standard before use.

Chart it once, find it every visit after

Zdrovia comes with starter chart templates for aesthetics, including a neurotoxin treatment record and a laser session record, and you can adapt any of them to match this one. Fill the chart on a tablet during the treatment, or let the AI Scribe complete it from what you say out loud. Products and lots dispensed from inventory land on the same patient record, and photos attach to the visit. Free for solo practitioners, with data stored in Canada.

Frequently asked questions

What should a laser hair removal treatment record include?

Skin type, hair colour and thickness, pre-treatment checks (sun exposure, medications, shaving, eye protection), the device and wavelength, spot size, fluence, pulse width and cooling for each area, the endpoint reached, and the skin's response. Include the session number so progress across the series is clear.

Why record a test patch?

A test patch shows how the client's skin responds to the planned settings before a full treatment. Recording the date, settings, and result protects the client and gives you a documented basis for the first session's settings.

This template is a documentation aid, not clinical guidance. It records decisions about dosing, depth, and settings; it doesn't make them. Documentation standards vary by province and profession, so adapt it to your regulatory college's requirements.
