Informed means understood

The document should explain the process in plain language: what sessions involve, that progress varies, and that therapy can be uncomfortable at times. It should also state the limits of confidentiality, such as risk of serious harm and legal requirements to report.

Include practicalities: fees, how cancellations work, how records are kept, and how to reach you or emergency services between sessions. Review it with the client rather than simply handing it over.
