Aesthetic Consultation Record Template
A printable record of an aesthetic consultation, capturing what the client wants, what you found, what you discussed, and what was agreed, so the plan and the conversation are both on file.
Document the conversation, not just the plan
A consultation is where expectations are set. Record the client's goals in their words, your assessment, the options you discussed including ones you declined to recommend, and the risks explained. If a client later says they were not told something, the consultation record is your answer.
End with what was agreed: treatments, sequence, and cost estimate if given. That turns the consultation into a plan the next visit can follow.
